Position Title: Direct Support Professional Program Assignment: Supported Living Immediate Supervisor: Supported Living Supervisor Minimum Qualifications: Abilities judged to be adequate for the successful provision of necessary support to people with disabilities. Applicants must be at least 18 years of age and have a high school diploma or GED. The ability to communicate clearly in oral and written forms to provide necessary support and produce a variety of documentation that meets or exceeds state and agency standards. Other Qualifications: A valid driver's license and acceptable driving record may be required from a risk management perspective. The physical ability to assist in lifting consumers may be required for some assignments. Position Summary: Responsible for working directly with individuals served at the assigned worksite(s) and providing the personal assistance and supervision required for individuals served to improve and maintain quality of life as outlined in Individual Service Plans (ISPs). Responsible for actively supporting individuals in all aspects of service provision in their homes and communities. Primary Responsibilities A. Service Provision 1. Maintains up-to-date, working knowledge of all ISPs and all related service documents. Implements all services as outlined in ISPs and related service documents. 2. Implements behavior and medical care plans safely and effectively. These plans may include physical interventions such as restraints. 3. Completes required worksite and classroom training according to established agency timelines. Attends staff meetings. 4. Makes recommendations to improve services based on firsthand knowledge acquired through the implementation of ISPs and experience with the individuals served. 5. Establishes positive relationships with individuals served and with external stakeholders as indicated by quality communication, commitment to individual rights and responsibilities, appropriate interactions, and the ability to create a balanced relationship with clear boundaries that allows for relating on fundamental levels. 6. Responsible for diligent monitoring and reporting of the physical and mental health of individuals served including any health changes and out-of-the-ordinary occurrences. 7. Keeps supervisor informed of important incidents, plans, and issues concerning individuals served. 8. Completes worksite-specific expectations according to training, direction from supervisor, and agency standards. 9. Acts as an advocate for individuals served. 10. Strictly adheres to confidentiality guidelines. B. Documentation 1. Responsible for writing progress notes for each shift worked that meet or exceed agency and state standards and meet established timelines. 2. Responsible for completing all applicable documentation according to agency standards and timelines connected to service provision for all shifts worked. This includes, but is not limited to: medication administration, finances of individuals served, incident reports, communications with stakeholders, scheduling matters, and shift checklists. 3. Responsible for making all corrections to documentation as directed by the supervisor. Secondary Responsibilities 1. Attends medical, psychiatric, and dental appointments with individuals as needed and properly documents each appointment. 2. Will be required to transport/accompany individuals served in the community as needed. 3. Maintains and/or assists with the financial records for the consumers and the work site where applicable. 4. May be required to assist with training other staff members.

Position Title: Supported Living Supervisor Program Assignment: Supported Living Immediate Supervisor: Supported Living Coordinator Minimum Qualifications: Preference of a BA or BS from an accredited college or university and/or a combination of education and experience totaling three years. A service provision background is essential. Supervisory experience is preferred. Other Qualifications: A valid driver's license and a driving record that is acceptable from a risk management perspective may be required. The ability to provide necessary support to people with disabilities within the assigned cluster group. Must be at least 18 years of age and have a high school diploma or GED. The physical ability to assist in lifting individuals may also be required for some assignments. The ability to communicate clearly in oral and written form and perform basic computer skills is essential. Position Summary: Responsible for the development and supervision of Direct Support Professionals and to ensure services are provided as stated in individual service plans. Facilitates Direct Support Professional training and ensures requirements are met. Is the primary contact for day-to-day issues regarding assigned supervisory and site caseload that may arise. Primary Responsibilities: A. Service Provision 1. Works with supported living staff, management, internal departments, and additional stakeholders to ensure service implementation as stated in individual service plans. 2. Schedules and facilitates monthly staff meetings to convey pertinent information in a timely manner. 3. Ensures the medical needs of individuals are met. This includes but is not limited to, ensuring timeliness of medical appointments, attending appointments, following up on noted medical concerns, ensuring current and accurate documentation and communication relating to the individual’s medical, etc. 4. Responsible for managing individual’s personal accounts and resources. Acts as a contact to accounting to resolve any financial concerns relating to assigned caseload. 5. Ensures all files at the site are kept current and maintained in accordance with established standards. 6. Review and approve service notes and additional documentation. 7. Provides service updates to Case Management and additional stakeholders when requested and communicates service needs and changes to appropriate parties in a timely manner. 8. Provides problem-solving and mediation for individuals and their ISP teams to resolve conflicts/foster relationships relative to the needs of the individual. 9. Collaborates with support departments to ensure departmental recommendations are implemented in a timely manner. 10. Completes additional assigned duties. The size of the supervision load and caseload will vary based on the specific duties assigned, which may change as agency need dictates. B. Supervision 1. Trains direct reports and substitute staff on all aspects of direct service provision. Responsible for ensuring direct reports provide high-quality service provision and implementation of service plan. 2. Performs and documents weekly out-of-ratio supervisory shifts during service provision to ensure direct reports are providing services in a manner that is consistent with the service plan, training, and agency core values and mission. 3. Completes and delivers direct report evaluations within established timelines. Provides ongoing timely feedback on performance and address performance concerns. 4. Responsible for all aspects of assigned supervisory caseload including development and discipline. Provides coaching/mentoring to support the development of essential leadership skills. 5. Proactively works with immediate supervisor and/or Human Resources to ensure direct reports adhere to established agency policies and procedures. C. Financial 1. Completes staff schedules that reflect appropriate ratios and are in line with established budgets every month. Responsible for working/filling open direct care shifts in a financially responsible manner. 2. Works specified number of budgeted direct care hours. 3. Review schedules/budgets with the Coordinator to ensure services are provided as determined in budgets. 4. Reviews and approves direct report clocked hours within established timelines 5. Review overtime report. Proactively works with supervisor to minimize overtime. 6. Completes and submits necessary payroll forms within designated timelines. 7. Proactively uses additional financial tools and reports to ensure responsible financial management of assigned caseload. 8. Works with the Finance Department to ensure services are provided within established guidelines. D. Commitment to the Mission, Vision, and Core Values 1. Establishes positive internal/external working relationships. 2. Share pertinent information with necessary parties on time. Uses respectful, open and honest communication. 3. Many position requirements occur in individual homes. Therefore, the ability to travel extensively throughout the community is required. 4. This position requires communication and interplay with several internal and external stakeholders. Therefore, professional and timely communication is considered a key component of this position. 5.
